Michigan Medicaid Health Plan Standardized Credentialing

 

Beginning October 1, 2026, the Michigan Department of Health and Human Services (MDHHS) and Michigan’s Medicaid Health Plans (MHPs) are implementing changes to how individual providers share information for credentialing and plan enrollment. These changes are intended to reduce administrative burden, improve timeliness, and standardize credentialing and enrollment processes across Michigan’s MHPs.

The resources below are designed to help providers understand and navigate the credentialing and contracting process with Michigan Medicaid Health Plans.

Review this document first for an overview of the credentialing and contracting process, including key instructions, terminology, definitions, and general requirements.

Below are the documents required for a complete application packet. They include:

  1. MAHP Provider Enrollment Form*
  2. MAHP Provider Enrollment Excel Roster for large practices with five or more providers*
  3. MAHP Provider Enrollment Practicing Locations Form (if multiple locations)
  4. MAHP Provider Enrollment Disclosure Questions (for Non-CAQH enrolled providers)
  5. MAHP Provider Enrollment Cultural Competency Attestation Form
  6. MAHP Provider Enrollment CSHCS Physician Attestation Form (if Applicable)
  7. MAHP Provider Enrollment Collaborating/Supervising Physician Form (if Applicable)

*This forms/roster initiates the credentialing process and includes the information necessary to enroll in the network if MHPs decide to contract with the provider or practice. Completion of this form does not guarantee the offer of a contract.
